Form Storage - No email address

Helle, newbie here trying to connect Contact Form to my email account, except I do not have an email address showing up in form storage as all the tutorials show.  Also note this form is in my footer. Any suggestions?

AOK- that makes a big difference.  Like I said, newbie here.  And now I can see that Form Block appears to be tuned for sending an email message (which I will use), and what I want is to build a 'subscribers list' and think I need to dig into 'email campaigns. Thank you Paul2009! Closed.
