How to obtain a signature on a form

I have a vendor application that needs to have a signature on it. What is the best way to incorporate that in a form? Is there a way?

What type of signature do you require? A typed signature is the easiest way to do this, you could also allow an image upload but that's not easy for the user. There are websites that allow for a sign option on documents, adobe and dropbox would be two, but what you use depends on your specific needs.
