Link form block information to a redirect page thank you message

Hello Everyone

I'm trying to create a redirect "thank you" pagef from a form in which i'd like to get some specific words (first name/second name/email/phone number) directly from the fields in the form.

The message would be something like this

Thank you for your request name  surname

 We will contact you in 24 hours at the email name@ddddddd or at the number: xxxxxxxxx
