Two Column Form Block

Hello, the form I have on this page is super long and i'm trying to make it shorter by having 2 columns versus 1 long list for everything. The 2 columns would be needed specifically for the following drop downs
1. Career
2. Money
3. Health
4. Friends and Family
5. Significant others/romance
6. Personal growth
7. Fun & recreation
8. Physical environment

You can try adding to Home > Design > custom Css
