Christmas Menu Booking Form

Hi, I've been asked to create a new Christmas menu page, like the link above, but with the ability to select food options in addition to the booking form used to reserve a table. My client wants the customer to be able to book a table online and select their parties food choice at the same time. Currently we use a downloadable pdf, see attached, but the problem with this is it's not always easy for the customer to return the form once printed and filled in. Please, any thoughts on how a customer can reserve a table and list their parties food options for: Starters, Mains, and Desserts? Thanks.

There is no block that can help you do this. Try looking for a 3rd party service or hire someone to handle it.
