Embedding Product Checkout Form into Product Page

Hello

I'm wondering if there's any way to add the product checkout form into the product page, before the add to cart? My client would like her customers to be able to write a card message & add delivery time/details prior to add to cart. Is this possible?

Thanks for the reply, but I'm specifically asking about the product checkout form, the one that will come up currently if you select add to cart, and is then processed with the order, all order information within the same order - not separately like a regular ss form on the product page. So my questions is can that form be embedded into the product page just above the add to cart button?
