How do I add an image in a code block that also links off-site?

I have a custom form that site visitors can use to join/renew membership. The form links to PayPal. The paypal logo image is broken BUT the text on the page is properly linked. Site visitors think that because the image link is broken they can't renew.

I can't figure out how to include the image AND get the link to work properly.

This is the code I have in place now:

The image I want to link to is here:

Can someone tell me what I need to do to get the image to display AND link to PayPal. I can provide full script if needed.

I'm intrigued to understand why you are using a custom form and a PayPal button when you can take the payments using the built-in Squarespace Commerce features and the built-in PayPal (or Stripe) integration.

This is a volunteer organization with multiple cooks in the kitchen. What you're proposing makes the most sense but I need to do this in the meantime. Great question!
