Imbedded Dubsado Lead Contact Form not maintaining mobile scale on Android, but works on Apple and desktop

Hi, on version 7.1 SS, added a code block to embed a Dubsado contact form. Was able to successfully embed the form (by clicking the "+" and adding code </>, form shows up great on Apple mobile and desktop but in testing on Android (Samsung, Motorola), the form initally shows up correctly but when you click on it to fill it in (ie/ populate your name) it doesn't maintain the mobile scaling and expands out to the point that all you can see is the corner of the box of one of the fields you would populate, like 2000% zoom in. You can pinch and scale it back down again to be able to see the form, but the minute you start to type again to populate the field, the same thing happens- mobile scaling is lost. Please note I only copied the HTML code that was provided on the dubsado form (pasted below). I didn't tweak or modify it at all. Also I haven't done anything with Ajax- in fact I am not even sure where to find that.

Of interest the other dubsado form I embedded (scheduling) works great on all devices (maintains scaleability).

Personally I think it's a problem with SS, but of course their Customer Care Team won't help because it's code. Any assistance anyone can provide would be greatly appreciated. I am new to SS and honeslty this is so instrumental that if I can't get this resolved will need to leave SS or dubsado. 😞

Thanks Derrick.  Zapier however is another cost; per month, if we have over 100 tasks/mo, as far as I understand it. Clearly would prefer to not have to pay yet another vendor.  As you probably can imagine small businesses don't have deep pockets for a lot of monthly fees.
