How to delete text on a 'Form template' and add placeholder text when it doesn't auto give you the option?

Hiya,

I am trying to delete the 'First Name' and 'Last Name' on a 'Form' template. It is auto on all of them and I don't want it as I would like to put placeholder text in those boxes instead. I also don't know how to add placeholder text when it doesn't auto give you the choice to- as I would like to add this in the 'Name' section on the 'Form' template.
