Trying to Create a Date Calculator on a Page

Hello, I am attempting to create an interactive element on a SS page that allows user input and then changes date information accordingly. 

For example, I would like a place where a user can input a date (let's say 1/1/2023) and then a set of dates auto populates based on that date (6 months before = , 8 weeks before = , 2 weeks before = ). 

I attempted to do this by embedding a published google sheets page but it looked awful and wasn't able to be manipulated from the site. Is there any option for something like this?
