How To Integrate A Checklist Into The Purchase Process?

Hello Everyone, 

I am selling a coaching session, and I want a mandatory checklist to the process. For example, when a person initiates a purchase (one a single session), I want to make sure they click 5 checkboxes, each with a disclaimer (such as the fact that I do not offer refunds) before I process for payment. Im not a coder, as an FYI.

You want to add this on Cart Page, Product Page or which page?

(Not possible on /checkout page)

Hola @tuanphan - it's more of a pre-cart qualifier, something that have to certify (by way of checking the checkboxes) before the cart populates.
