how i redirect user to pgae with input

hello everyone

i'm new here i wish someone help me

i want to add text input with submit button and when user insert the code in it,, it will redirect to another page depends on the code

for example if user insert "hello77" in the text input it will redirect to "www.site.com/hello77"
