Embedding Survey Monkey Java Script - bad code?

I want to embed a Survey Monkey form on a Squarespace page. I copied the javascript code for the form from Survey Monkey, created a page, added a code box and added the code. Javascript was checked. However Squarespace only displays the code when I close the code window.  It will not display the form.

Can anyone shed any light on this or recommendations?

I imagine the code provided is HTML, not JavaScript (JS). If you see <script> then it's HTML, which may have JS (or a link to JS) within it.

Change the Code Block setting to HTML and try again.
