Turn TEXT into a POPUP LIGHTBOX FORM

Hello,

Can someone help me with the code to make this work.

On my website LARecoveryconnect.org, the button "CONNECT WITH US" on the upper left is working well for me.  This is a contact form that is a lightbox button.

I want the "Contact" text in the footer to do exactly what the button is accomplishing. 

What coding do I use to make this work?

You can add Form Block >> Enable Lightbox button > Then we will give the code to turn button style to text link style.
