Error message in Form using 'Survey' field

I created a form and used the 'survey' form field and added 4 options. I have all the other information entered correctly but I keep getting this error when I submit the form. Each of the options has one item selected. I have tried troubleshooting this for hours and can't find anything on it. Appreciate any help/assistance. 

So I just removed 2 options and kept just 2 for the 'survey' form field and it worked. So does this mean that you can only have 2 options per Survey form field ?

I also am having HUGE issues getting the SURVEY component to work for me without the "INVALID QUESTION" block--and I only have the "original" 2 survey questions in play right now. I don't understand how to make it work!

I am also seeing this "Invalid Question" error message with the Survey input. I am going to replace w/ a different element. I'll replace the opening question with a 'Line' or 'Text' element. I need to figure out if I want to use 'Radio' or 'Select' (drop-down). I think 'Select' since it takes up less space... or reframe to have it be a "Select All That Apply" question....

I have encountered the same issue with the survey form with only one question. Is there still no solution to this?
