1. Add a Service Product for the Charge Notice Payment.
2. When adding the product:
   • Add a Custom Product Form that prompts for these two fields.
   • Change the Add to Cart button to something more appropriate (Pay now?). To do this, Click Custom Button, then switch the Custom Button Label toggle on and edit the text in the Label field.
3. Add a Product Block to your page that links to the new service product. Disable all display options except the button.
4. Enable Express Checkout.

Note: This assumes you are not already using the site to sell products or services.
