How to enhance a form link in scheduling

I am using scheduling to schedule and pay for teaching services.  I want to attach a terms and conditions form via URL link to the Appointment Type with the Forms feature.  In the Form Description Field, I am including a URL link to my terms and conditions. 

The Squarespace Help on this feature states - "You can include links in the form description or question. Pasting a URL, including the http://, into any question or form description field will create a clickable link. You can also use HTML to create a more polished link in the form description."

Can you provide an example of the HTML to improve the look of the "http://..." link the customer sees?
