Card layout for forms

I have a design to create a customised contact form and it is required to have a card like design with a background image. And the form is not in the regular input design. 

I can only see limited customisation levels for sections and forms.

Is there any way I can implement this successfully?

You can create something that looks like this by writing some CSS to change the styles. The first step is to create the form using a Form Block.
