How to add image to custom check out form?

I have made a custom checkout form for my shirting website in development. In that there is a question where I ask "Which body type you associate with yourself?" and for this I need a photo to display before the options. I looked online for a solution and added the image in Design>Custom CSS and copied the link into the form. But it didn't work. Please help me out. I don't know coding.

Custom checkout form is not accept any custom code injection, you can only change how the product form looking

Oh. Thanks for explaining it. Another solution that comes to mind is to redirect checkout button on shopping cart page to my custom form instead of going directly to checkout. Can you help me with the coding?

What kind of form you already have?

This the form page link. I wish the shopping cart button to redirect to this and the checkout button at the bottom will bring it to checkout (payment, address,etc).
