How do I add a text box on a product page?

Hello. Is it possible to add a text box so that a customer can leave a note on a product page? I sometimes offer products that can be customized, so it would be great if a customer could leave the note right on the product page and not in the note area at checkout.

You can set up a product form, it will show up after user click on add to cart button
