Multiple, complex variants with varying costs

I'm designing this site for a printer who sells apparel to teams... I have various variants for sizes, but need to add another variant with price adjustment to add the option of name and number to the back of the jerseys, which would increase the cost.

Is there a way to add the variant in a column, or must I list out in rows each possible variant? Listing in rows seems insane to ask a buyer to scroll down each size and yes/no to name and number on the back.

Meaning: select your jersey size with a drop down, print your name with a drop down and price adjustment, print your number with a drop down and price adjustment.

HELP!
