Add-on Checkbox on Product Page

I'd like to offer 'extras' before item adds to the cart.

For example, a checkbox that would add to the price of the item:
☐ PDF proof - ADD $3
☐ Paper Upgrade - ADD $10
☐ Rush Charge - ADD $20

Any ideas on this?

You can only add these option as variants

Thank you, this is very helpful. I understand how to make that work. 

My set up is pretty complex to do manually, so I'm using the template. Do you know if there is a way to include more than the 3 variant columns that are set up in the template? I think I'd need about 6.

Could you use csv import?

That's what I'm referring to, sorry. 

The csv template only has 3 columns. I think if I add columns in there (Option Name 4, Option Value 4), it will probably not work right...

...actually, that DOES work.
