How do I add a widget from Pabau to squarespace for lead capture?

I need to add a widget to my website.  I don't know where, or what wording to use but I do have the code from pabau. 

I tried to add this to my pop up but clearly that's not the right place, I have no idea how to do these things.

Can anyone advise: 

Where a widget should go for lead capture please? 

How to add the widget to my site and what wording etc to use for gaining these leads etc
