EmailMeForm code not working

I am trying to embed code from company called

EmailMeForm, using a custom code block and it is not working.  Instructions seem simple and I feel I am doing it correctly.

Has anyone had success with this?  I have tried all formatting options (HTML, JavaScript, Markdown, etc.) and none work.

Screenshot included.  My site is on the business plan but not published yet.

Do everything you show in your pic but don't check the Display Source option. What happens then?

You can share your site for us to help you. Set up a site-wide password. Post the password here.

Thanks for your suggestion.  The Display Source option is automatic and I cannot turn it off when checking the Javascript option.  I feel a bit uncomfortable opening up my site with a password for all to see.  Does this allow anyone to make changes?  Couldn't that open me up to potentially bad things?
