Form box rearrange

Hi, 

Does anyone know how to rearrange the information bar on the form?

I want the subject bar to be beside the email and not below.  How do I bring it up?

Also, how do I edit the font size of the submit button?

Currently it looks like this:

But, I want it to look like this:

Can someone help me out here please?
