How to send a customer follow-up e-mail with a form that is pre-filled with his order # and product #'s.

I need to have a way to communicate with the customer regarding their recent purchase.  I do not see an existing way to send an e-mail through Sq.Space after the service has been fulfilled.  I would like the order number, event name, and service items to be automatically generated with a section for them to respond next to each item.  

I also need to track these responses in a Google sheet.  Can someone walk me through the best way to do this?
