Best way to do a class video feedback page

I'm working on a site for a piano studio and they would like to Class specific pages where students can upload their performance videos and others can provide feedback. What's the best way to accomplish? It doesn't look like you add a "file upload" field to a form.

Any ideas are welcome, thank you in advance!

Did you ever figure this out? I am doing the same with my bass guitar lessons website and want students to upload progress so I can critique!
