Customize placement of Form Checkboxes

Hello,

I would like to see how I can get these form checkbox sections to line up next to each other. 

I would like the 'WELLNESS', 'PSYCHOLOGY' & 'SPIRITUALITY' to be side by side. Then below the 'OTHER' and the 'ADD YOUR OWN'.
