Music license form - Auto Generated PDF

Hello Forum! I'm curious wether someone knows how one would go about creating a Music License Form similar to the one provided in my Site URL field. Where what the visitor inserts into the form automatically gets added into a PDF, that once downloaded also features the current date of download.

I have very little knowledge of coding and without knowing the potential complexity of this I'd greatly appreciate if someone could push me in the right direction through some valuable links etc or help out with code. As there's a growing amount of independent music producers and artists out there today, hopefully this thread might be of value to more people as well.

I talked with Squarespace support and was notified about Wufoo - Went through their templates and on a first rather quick glance it doesn't seem like they offer templates for this sort of thing. But I've sent their customer service an email about it.
