index pages not displaying consistently throughout site

For testing purposes, I've created a test page. It's called "test page." When you click on it, you see the content, page title, page description, and the index below. On my Remnant page, I have linked the test page to the first image (called "test button"). Clicking the first image will also direct the user to the test page.

My problem is that when I access the test page using the button, I don't see the page's title or description. I also noticed that when I access the test page via the button, the url is different.

How do link to index pages and retain the page's title and description?
