Add to Cart button disappearing after form

Hey guys, I teach private guitar lessons and I want people to fill out extra as a part of the purchase. The problem is that when I add the form, the "purchase/add to cart" button goes away, so then you're just stuck with the page. If I turn the form off, there is no issue.

Edit: I also tried filling out all of the required fields and still nothing. 

Edit 2: I took all the required fields off and the purchase button showed up. Whether the site is listed/unlisted doesn't seem to make a difference. This seems to be an intermittent problem because sometimes it shows us but most of the time it doesn't: both with the editor's link and on the regular site. 

Add to Home > design > custom CSS

    border: 1px solid currentColor;
}

Thanks for the quick reply!! The button showed up. Is there any way to make it a little less dark?

You can't read what it says unless you over over it. Do you think it was just colored in white before? Appreciate the help!!
