iFrame Form not loading properly in live site

I have created a form in ClickUp that is supposed to be embedded via an iframe. 

When I put the code in as an HTML code block, I can see the instructions of the form when editing, then I get a Script Disabled message (which, from what I can tell, is standard protocol when in edit mode). 

When I go to the live page, the form instructions show; however, none of the fields show. It only displays instructions and the submit button. The form URL is being served over https.

I'm not seeing any error messages in the developer console when I load the page.

When I open the code block in the editor, the closing iframe tag is highlighted yellow, which leads me to think there is something stopping the iFrame from displaying properly. I know the form is public because I can see the form instructions when I browse to it.

I've confirmed that the form is live and publicly available (I can navigate in a browser to the provided URL) without issue.

Any thoughts on what might be preventing the fields in the form from displaying completely in the iframe? I tried removing the style element just in case the HTML was not wanting to render the inline styling, but that didn't have any effect either.
