Checkout Form Description In Email

 I have setup a form for checkout on my Squarepace site. The form is a waiver with the user having to check Yes, I agree upon checkout with a digital product. When this is sent to me after checkout, the description isn't visible in the email confirmation or the order. Is it possible to have the description of each form field be included in the checkout email? I have attached an example below. The description in the checkbox form is not sent to me, only the "yes, I agree" and the title "COVID-19 Waiver." Is it possible to include the disclaimer in the description box?

The solution is to change the checkbox value to "I have read and agree to...." and remove the description. That way, the assertion will appear in the email correctly.
