Social Media Live Follower Count

I'm trying to create a live social media follower count for each site at the bottom of my site. See screenshot for example.

Any ideas on how this could be accomplished? Just spent the whole evening scouring the web to no avail. Let me know if you have any ideas! Thank you.

Then share url, We can give the CSS to put them in a line.
