Making a calculator on my page to help in the coronavirus pandemic

Question

Hi, I want to make a calculator on my website to help other physicians with calculations for the coronavirus pandemic. I upgraded my plan to Business and tried inserting a javascript code block, but it won't let me uncheck "Display Source." So the code block won't run.

I tried used Squarespace's default forms but that just emails results to me. I want to use form results to calculate something directly on the page and then display results there, similar to MDCalc: for example, https://www.mdcalc.com/heart-score-major-cardiac-events (but it doesn't have to be as fancy/pretty)

I thought this would be easier. What is the best way to do this? Thank you for your help.
