Embedded script not loading without a page refresh

I'm having trouble getting an embedded script to load on my page without the user having to hit the refresh button.

The page is  https://www.solarshare.ie/arrange-roof-demo  and the embedded script is below the paragraph of text and the Google maps image.  It should be showing a Meeting Booking form, but it only displays the form after the user hits the refresh page.

Any ideas?
