G Suite Drive Integration - allow uploading from site

I'm trying to see if there's a way I can have a site visitor fill out a detailed contact form that includes a way for them to upload files, on that same form, and both be sent to my G Suite Drive (as both form content that sync's to a document in my G Suite Drive and as the actual files that they'll upload to a shared G Suite Drive folder). I'm looking for a solution that looks like a detailed contact form with possibly a button that triggers a popup dialog for selecting files to upload and when they hit Submit, it all sync's to my G Suite Drive and I give them a thank you message with additional info. I'd love any suggestions for some solution that gets me close to this and keeps everything done on my site vs an external user experience. Thanks!
