2 stores different Checkout redirect

Hey guys,

I've recently set up my store (i run a retreat business), after my customer checks out i wish for the confirmation page to redirect to a form (either squarespace or google) where they can answer a questionnaire and fill out their requirements for the retreat they have just purchased. Each retreat is different. Thus needing a different form.

I injected the code into the order confirmation page although it appears this is only good for one form/page. I need to have a unique form redirect for each individual retreat 

Can someone guide me through this please?
