How can I make it so that when you click on an image, it turns into a form.

I'm trying to set up a capability so that when someone clicks on an image, it pulls up a form, just like the button does in lightbox mode. How can I do something like this?

I've used SQSP Themes Lightbox Anything (aff) plugin to accomplish this. It works great for any kind of lightbox work you want to do.
