Anyone know how to implement a conditional form?

Question

Posted (edited)

I have a form with radio buttons. If a person answers/clicks "yes" to the first question then I want them to see question #2. If the person answers "no" then I want them to see a message showing they are not qualified. So depending on how the user clicks, makes a selection will determine what that user sees in the form.

Thank you.

I attempted this code, but it did not work:

This isn’t recommended on Squarespace Form Blocks. It’s best to embed a third-party form that includes this feature, one of the best is TypeForm.

If anyone's response has helped you, show them some love by clicking 'Like' like.jpg.6f2856e82374ca8b5041fff9ee27eef4.jpg. If their response didn't help, ask for more help, or wait for users to chime in.

Definitely research some third-party plugins. I have not done this yet so I don't have any recommendations.
