  1. Site URL: https://www.bvpt.com.au/get-started Hi everyone, I have recently embedded a multi page Jotform via a code block on my site. It has approximately 3 "Next" buttons at the end of each page, with a submit button. This integration works perfectly on desktop and tablet. It also appears fine on my mobile (iphone) on Chrome and Brave. A client showed me the same page today (from Samsung/Chrome) and the bottom of the form was cut off, and the Next button was not accessible from page 2 of the form onwards. I'm guessing its a margin issue, but not really sure. Any suggestions?
  2. Site URL: https://www.drneilkudler.com/questionnaire/ I am at my wits end with this form. I implemented this JotForm a few months ago and recently when patients come to the questionnaire page the form will pop up just for a second and disappear completely. When I go into edit mode there is a little "scripts disabled" warning at the bottom of the block. I've tried just using the iFrame with out the javacript coding. I've made sure that all of the web addresses in the form code start with https. I've followed just about every tutorial out there. Does anyone have any insight as to what is going on here? The address is: https://www.drneilkudler.com/questionnaire We're running on 7.1 so there is no way to change any Ajax settings. The code block itself is: <iframe id="JotFormIFrame-201435079579059" title="Pre-Visit Questionnaire" onload="window.parent.scrollTo(0,0)" allowtransparency="true" allowfullscreen="true" allow="geolocation; microphone; camera" src="https://hipaa.jotform.com/201435079579059" frameborder="0" style=" min-width: 100%; height:539px; border:none;" scrolling="no" > </iframe> <script type="text/javascript"> var ifr = document.getElementById("JotFormIFrame-201435079579059"); if(window.location.href && window.location.href.indexOf("?") > -1) { var get = window.location.href.substr(window.location.href.indexOf("?") + 1); if(ifr && get.length > 0) { var src = ifr.src; src = src.indexOf("?") > -1 ? src + "&" + get : src + "?" + get; ifr.src = src; } } window.handleIFrameMessage = function(e) { if (typeof e.data === 'object') { return; } var args = e.data.split(":"); if (args.length > 2) { iframe = document.getElementById("JotFormIFrame-" + args[(args.length - 1)]); } else { iframe = document.getElementById("JotFormIFrame"); } if (!iframe) { return; } switch (args[0]) { case "scrollIntoView": iframe.scrollIntoView(); break; case "setHeight": iframe.style.height = args[1] + "px"; break; case "collapseErrorPage": if (iframe.clientHeight > window.innerHeight) { iframe.style.height = window.innerHeight + "px"; } break; case "reloadPage": window.location.reload(); break; case "loadScript": var src = args[1]; if (args.length > 3) { src = args[1] + ':' + args[2]; } var script = document.createElement('script'); script.src = src; script.type = 'text/javascript'; document.body.appendChild(script); break; case "exitFullscreen": if (window.document.exitFullscreen) window.document.exitFullscreen(); else if (window.document.mozCancelFullScreen) window.document.mozCancelFullScreen(); else if (window.document.mozCancelFullscreen) window.document.mozCancelFullScreen(); else if (window.document.webkitExitFullscreen) window.document.webkitExitFullscreen(); else if (window.document.msExitFullscreen) window.document.msExitFullscreen(); break; } var isJotForm = (e.origin.indexOf("jotform") > -1) ? true : false; if(isJotForm && "contentWindow" in iframe && "postMessage" in iframe.contentWindow) { var urls = {"docurl":encodeURIComponent(document.URL),"referrer":encodeURIComponent(document.referrer)}; iframe.contentWindow.postMessage(JSON.stringify({"type":"urls","value":urls}), "*"); } }; if (window.addEventListener) { window.addEventListener("message", handleIFrameMessage, false); } else if (window.attachEvent) { window.attachEvent("onmessage", handleIFrameMessage); } </script>
  3. Site URL: https://www.upleftfinancial.com/risk-tolerance-questionnaire Hello, I am using Jotform code embedded into my site to created a submit-able form. The code is working fine on desktop and tablet views, however when using a mobile device the content and all the text block disappear. I have tried disabling the AJAX as squarespace help suggested however it has not made a difference. This is an important componant since so many people do everything on their phones these days.
  4. I have an embedded jotform questionnaire on a page. When a user finishes the questionnaire, they should be redirected to a booking page (which has been built with Squarespace's scheduling tool), but that's not working - the user stays in the same page upon completion of the questionnaire. To enable this functionality on Jotform, I selected the url of the booking page as a redirect when a user finishes the questionnaire. I know that works because I have the same form embedded on a Wix website and it redirects correctly. For additional context: I'm still on squarespace's free trial, and my page is published on a squarespace page (no domain connected yet). Thank you! Nicola
  5. Hello, I am using Memberspace for managing member and Jotform to submit member request in a Memberspace protected page. I am trying to pass member data specifically First Name, Last Name, email, and cell phone number (this is a custom field in Memberspace regsitration form), but have not been successful. This is what I am doing: I code injected the following in the header (setting > advance > code injection) below the required Memberspace install script: <script> var namaDepan; var namaBelakang; var eMail; var noHP; (function() { MemberSpace.onReady = MemberSpace.onReady || []; MemberSpace.onReady.push(function(args) { if (args.member) { /* Your custom code goes here */ namaDepan = args.member.firstName; namaBelakang=args.member.lastName; eMail=args.member.email; noHP=args.member.customField11284; document.getElementById("namadepan").innerHTML = "<p> Halo " + namaDepan + ", apa kabar?</p>"; /* to test whether data can be passed*/ } }); }()); </script> With the above code I can write the first name of a member anywhere in the page as long as I marked the location with id="namadepan". However when I try to pass the variable {namaDepan} to Jotform code it appears the value of {namaDepan} is not passed. This is what I do with Jorform code (in the iframe part) - the Jotform code is written in a Code Block in the page: <iframe id="JotFormIFrame-201448534058051" title="Barang Dibutuhkan" onload="window.parent.scrollTo(0,0)" allowtransparency="true" allowfullscreen="true" allow="geolocation; microphone; camera" src="https://form.jotform.com/201448534058051?name[first]="+namaDepan+"" frameborder="0" style=" min-width: 100%; height:539px; border:none;" scrolling="no" > </iframe> However, if in the statement src="https://form.jotform.com/201448534058051?name[first]="+namaDepan+"" if I change it into src="https://form.jotform.com/201448534058051?name[first]=John" it works. So if I use just manual inputed value it works, but if I try to pass a variable it wont work. Please if anybody have any way to solve this issue please let me know. Thank you all in advance. Best regards, Wantya
  6. I am using source code (not iframes or scripts) from Jotform to place forms in code blocks on my page, but whenever I do this on Squarespace 7.1 (not a problem in earlier versions), I now see a litany of console errors when I load the page, and I lose the ability to edit the page, forcing me to delete the page to redo my work. It also removes images from the page, though the div remains. I have jQuery placed in the global site header so I have access to its selectors. When I try to use jquery selectors on the console on jotform pages, they also break. Below are the errors I see in console: Any advice anyone might have to solve this would really help me out. I appreciate any guidance I can receive.
