Can a scorecard be built in Squarespace? (Example Included)

Is it possible to create a scorecard-like form within Squarespace? Similar to the image attached There are 2 teams and viewers can score each round like of a boxing match? I've tried creating a form with radio buttons but I quickly found out it wouldn't work. I would like to customize the scorecard so I could include an emoji instead of the checkmark which would appear for the users choice. Any suggestion would be greatly appreciated. -
I'd like to add an image upload to the product form when you hit add to cart. I have tried to implement squarewebsites uploader but it only allows 10Mb and dev won't open it up more. I cannot seem to find an option that integrates in the same way with having the form pop up after adding product to cart. I am now trying to utilize Jotform but I cannot find a way to get it working with product form. I think Squarespace needs to have an option for an upload to a cloud service like google or dropbox for the commerce plan, having to utilize a third party for a form to get an upload button puts cust -

How do I create an autoresponse to the messages that come through from the Contact Us form? I read a post that said to download Zapier but does that work for non-gmail accounts? The response would need to come to a business account. Thank you! -

Hi, i’d like to know if there is a way to change the sender and subject lines from when customers submit forms. Right now all submissions show ‘Squarespace’ as sender and ‘Form Submission - xxx’ as Subject. is there anyway to change this? I’m willing to even pay for a third-party solution but I cannot find any. Thanks in advance for your replies.

Hi all I've had an enquiry where the client wants to a series of questions to be shown to the user, each question will narrow down the products available in a database and at the end the user will be presented with a narrowed down amount of products. Originally I had linked an image from an outside source since I couldn't figure out how to do it within Squarespace. It worked fine for a long while, but now I'm finding that image doesn't display. I am looking for a way to change the format of dates in blog posts (contained in the meta field of blog posts). I am developing a Swedish language website and would therefor like to change months from text to numeric (for example: change “March” to “03”) or from English to Swedish (change “March” to “mars”). Hello, I have products set up in my store which have forms attached. Ordinarily, when you set up a form there's the tab to change the email that it's linked too and to link google drive to create a document which holds the data. I can't see a way to do this for the product forms. Can anyone point me in the right direction? Thank you. -

Ok, I hope someone can help! I can see in my analytics that forms were submitted, however I did not receive any of them. Long story short, my email was not set up correctly which was my storage method. I am wondering if there is anyway that Squarespace has access to these forms. Wishful thinking.

Hey Everyone! Is there a 3rd party service (or a way) to integrate a petition sign-up form on a Squarespace site? The basic functionality that I want it to possess is: Form fields that allow me to collect name, location, zip-code, display the order of their sign-up (e.g., submission 1, 2, 3, etc.) Submit button Maybe the data is also captured and easily recorded in a backend spreadsheet (e.g., Google Spreadsheet) The data is displayed on the Squarespace page one the form is submit and the page is refreshed Any thoughts on how to make this happen? When I export the orders, none of the information from the custom form is included in the export to CSV. I see the information in the confirmation email and also in the actual order. Is it possible to export the custom form information that is collected from an order to a google sheet?

When we refresh the page, the logo does show up, but the custom form, located at the bottom of the home page and also in the contact us page, disappears.
