My client contacted me about this issue too. This seems to be an internal Squarespace page. Oddly enough, I tested in Browserstack, and the checkout form is different. I think this particular form appears if you are signed in to the phone, or possibly if you have signed up to Apple pay. I think it's probably most people on iOS. The Browserstack testing might not be accurate. Seems like a Squarespace UI bug.
