Increase placeholder font size? Or move form titles into the box it corresponds to?

Hi all, I am trying to get a form that looks like this... How do I get the form placeholder text font increased? The placeholder font-size css isn't working. Also how can I move the form titles in line with the input boxes, then if I can't increase the placeholder font-size I can just take it out.
