I have tried to recreate the page and also create from a duplicate page and still no luck. Any ideas?

We have moved to a new online scheduler, and I need to swap the custom code on this page https://www.signaturesmiles-nc.com/request-appointment with the newest vendor's custom code for their scheduling widget. I am having no luck at all and wondering if maybe someone could advise or help? Kind of new to this so appreciate any suggestions.
