I found the solution on this page: https://support.squarespace.com/hc/en-us/articles/206540677-Exporting-orders On step 3 of the order export, select a specific product, then check the option to "Include product form data." The exported file will show form responses for each order. The only downfall is you have to do one product at a time during the export. Hope this helps!
