Custom Product Forms cannot affect the cost of a product and can only appear after the Add to Cart button is clicked.
However, you can add a product variant for the different style and assign it a different price. The variant option will appear beside the Add to Cart button.

I recommend reducing the number of navigation items, as nine is a lot for visitors to process, which means they are likely to scan past some important ones.
'Home' can be moved to UNLINKED, as clicking the site logo will take visitors home.
